He had his thumb to the top of the plunger about to press on it. Angel's body would shake but he would hold her in place and the way she did that got him thinking that she was desperate, wanted to avoid what was about to happen so as he tightened his grip he thought that it was over but also the need to hasten this before he heals.
The scan the mini drone did was what he used to judge how fast she could heal and so he wasn't expecting her to do something where she controls her healing process and forces it to happen in a single second.
